1. Appointments

Appointments can be made by phone, online booking, or in person. By booking an appointment with The Clinic Quairading, you agree to these Terms and Conditions.

2. Fees and Payment

Payment is required at the time of consultation unless otherwise arranged. We have HICAPS facilities available for on-the-spot private health insurance claims.

3. Cancellation and Rescheduling Policy

If you need to cancel or reschedule your appointment, you must provide at least 24 hours’ notice prior to your scheduled appointment time.

Cancellations or rescheduling requests made with less than 24 hours’ notice may incur a cancellation fee. Failure to attend an appointment without notice may result in a full consultation fee being charged.

4. Late Arrivals

If you arrive late, your appointment time may be reduced to avoid delaying other patients. Full consultation fees may still apply.

5. Health Information

You agree to provide accurate and complete health information. The Clinic Quairading is not responsible for issues arising from incomplete or inaccurate information provided by patients.

6. Privacy

Your personal information will be handled in accordance with applicable privacy laws in Western Australia. Health information is kept confidential and used only for the purpose of providing care and related administrative functions.

7. Liability

While all care is taken to provide professional chiropractic services, results may vary between individuals. The Clinic Quairading is not liable for outcomes outside of reasonable professional control.

Pilates Class Waiver, Photography Consent & Cancellation Policy

By booking and participating in a Pilates class at The Clinic Quairading, I acknowledge and agree to the following terms and conditions:

Assumption of Risk

I understand that Pilates involves physical movement, strength, flexibility, and balance activities that may carry a risk of injury. I voluntarily choose to participate and assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.

Health & Medical Clearance

I confirm that I am physically able to participate in Pilates classes and have no medical conditions, injuries, or limitations that would prevent safe participation, or I have disclosed these to the instructor prior to class.

I understand it is my responsibility to consult a physician if I have any concerns regarding my health or fitness level.

Release of Liability

I hereby release and hold harmless The Clinic Quairading, its owners, instructors, employees, and contractors from any and all claims, liabilities, demands, or causes of action arising from or related to my participation in classes, including those caused by negligence, to the fullest extent permitted by law.

Personal Responsibility

I agree to follow instructor guidance, listen to my body, and stop exercising if I experience pain, dizziness, or discomfort beyond normal exercise exertion.

Photography & Video Consent

I understand that photographs and/or video recordings may be taken during classes for marketing, educational, or promotional purposes (including social media, website, and advertising).

By booking a class, I grant The Clinic Quairading permission to use images or recordings in which I may appear without compensation.

If I do not wish to be photographed or recorded, I understand it is my responsibility to inform the instructor or studio staff prior to class.

Cancellation Policy

I understand and agree to the following cancellation policy:

  • Cancellations must be made at least 12 hours before the scheduled class start time.

  • Late cancellations or no-shows (less than 12 hours’ notice) will result in the loss of the class credit and/or a full charge for the class.

  • Exceptions may be made at the studio’s discretion.

Class Passes & Expiry

  • 10 Class Pass is valid for six (6) months from the date of purchase. We encourage clients to use their classes within this period to receive the full benefit of their pass.

  • 2 Class Pass is valid for one (1) week from the date the first class is redeemed. The second class must be used within 7 days of attending the first class.

  • Class passes are only for use by the original purchaser and are non-transferable and non-refundable.

  • Purchasing or holding a class pass does not automatically reserve a place in a class. All classes operate on a first-come, first-served basis, and a spot is secured once a booking has been confirmed.

  • We understand that unexpected medical situations can arise. In cases of injury, illness, or medical treatment, please contact The Clinic as soon as possible.

Acknowledgment

By booking a class, I acknowledge that I have read, understood, and agree to this waiver, photography consent, cancellation policy, and class pass terms.
